I certify that I have fully and accurately answered all questions and have given all information requested in this application for employment, and I understand that any wrong or incomplete information on the form may disqualify me for futher consideration for employment or, if discovered after I am hired, may be grounds for my immediate dismissal. I understand that all such information is subject to verification by the S&H Powerline Construction, Inc., and hereby give my consent to the Company to investigate my background and qualifications using any means, sources, and outside investigators at its disposal. I agree to undergo any type of drug and/or alcohol testing that the Company may require at any time. I understand that submission of this application does not necessarily mean that I will be hired, and that if I am hired, my employment will be at will, and either I or the Company may terminate my employment at any time, with or without notice or reason. If I am hired, I agree that, as a 90-day temporary employee of S&H Powerline Construction, Inc., I will be in attendance and on time every day, including Saturdays that may be needed to help fulfill a 40 hour work week. I agree to learn about the job I do and the safety procedures that go with it. I further agree to observe the rules of common courtesy to my fellow employees and our customers. By signing this form, I give my permission to S&H Powerline Construction, Inc. to investigate my driving record and references, unless specified and with good reason. Failure to have a valid driver's license may be grounds for termination. I understand as an employee I am subject to drug/alcohol testing.

I agree to abide by these rules after my probationary period is complete.

Under the provisions of the Fair Credit Reporting Act, 15 USC, Section 1.681 et sq., the American with Disabilities Act and all applicable federal, state, and local laws, I hereby authorize and permit S&H Powerline Construction to obtain a consumer report and/or investigative consumer report, which may include the following:

1. 1. My employments records;
2. 2. Records concerning any driving, criminal history, credit history, civil record, workers' compensation (post-offer only) and drug testing;
3. 3. (For truck drivers only) In accordance with the Department of Transportation Motor Carrier Safety Regulations, Section 382.413, information concerning alcohol and controlled substances for the past 2 years;
4. 4. Verification of my academic and/or professional credentials; and information and/or copies of documents from any military service records.

I understand that an "investigative consumer report" may include information as to my character, general reputation, personal characteristics, and mode of living which may be obtained by interviews with individuals with whom I am acquainted or who may have knowledge concerning any such items of information.

I agree that a copy of this authorization has the same effect as an original.

I hereby release and hold harmless any person, firm, or entity that discloses matters in accordance with this authorization, as well as S&H Powerline Construction from liability that might otherwise result from the request for use of and/or disclosure of any or all of the foregoing information.

I understand and acknowledge that under provision of the Fair Credit Reporting Act I may request a copy of any consumer report from the consumer reporting agency that compiled the report, after I have provided proper identification.

I hereby authorize S&H Powerline Construction to obtain and prepare an investigative consumer report as set forth above, as part of its investigation of my employment application.
